Out of Hospital (Home) Births

If you had an out of hospital birth you must register your baby's birth. This is done by appointment only. Please call our office at 925-313-1125 to obtain the forms and information you need to schedule an appointment.

There is no fee to register the baby, however a certified birth certificate is $36.00
