Environmental Health Complaint Report Form

If you need to file a complaint regarding a suspected foodborne / waterborne illness, do not use this email form. Please contact us directly via telephone at 925-608-5500 as soon as possible and an Environmental Health representative will take your report.

While completely anonymous complaints are accepted, inclusion of your name and contact information is highly recommended should staff members need to contact complainants to gather more detailed information regarding the nature of the alleged complaint. Additionally, staff would like accurate contact information in order to follow-up with complainants at the conclusion of our investigation to inform complainants of the result and any action(s) taken.
