Change my Mental Health Provider
If you have been unable to resolve a problem with your service provider, you may request a change of provider. We want to provide quality care for every patient, and part of that is ensuring you are working with the right provider for you.
If Your Provider Works Out of County Clinics
If you see a provider at one of Contra Costa Health's mental health clinics, you can request a change of provider by completing a form. You can pick up a form at the front desk of any mental health clinic or print it out online.
Please note that requesting a change of provider does not guarantee that your request will be granted. We review all requests seriously, and we will respond to you within 10 working days.
If Your Provider Does Not Work Out of a County Clinic
If you are on Medi-Cal and see a mental health care provider who does not practice at one of the County's health clinics, we can still help you switch providers. Please call the 24/7 access line at 888-678-7277 to start the process.
