Prenatal Care Guidance (PCG) aims to promote optimal pregnancy health and birth outcomes by providing brief intervention in-home support and educational services to low-income pregnant women residing in Contra Costa County. PCG offers up to three home visits for a pregnant woman and provides education, support and resources to ensure that all the family’s immediate needs are met.

Who is Eligible for PCG?

PCG offers Short-term and Long-term case management home visits for pregnant people and provides education, support and resources to ensure that all the family's needs are met.

What Services Does PCG Provide?

A team of Community Health Worker Specialists (CHWS) provide the following services:

  • Assist women and their children in gaining access to health coverage and health care
  • Connect women to prenatal providers
  • Promote women's health during, and between pregnancies
  • Educate and support families about pregnancy and parenting
  • Link families to appropriate resources and services

    All services are free and provided in English and Spanish regardless of immigration status.

How Can a Woman Access PCG Services?

Pregnant women can be referred by their prenatal care or Healthy Start Program provider.

Pregnant women can also self-refer by calling 1-800-696-9644 or completing the FMCH Referral Form.

PCG serves all of Contra Costa County.

The PCG interdisciplinary team consists of Community Health Worker Specialists, Administrative Support and a Program Manager.
