Primary Care Provider Change

To change your CCHP Primary Care Provider (PCP), you can:

• Fill out and submit the online form below  OR
• Print, fill out, and fax or mail this pdf form to CCHP  OR
• Call Member Services at 1-877-661-6230 (Option 2) (TTY 711), Monday - Friday, 8am - 5pm.

If you need to change Primary Care Providers for other members of the family, complete separate requests for each person.

Note: all fields must be completed.

Online and faxed or mailed requests are processed in one business day after receipt.  If there are issues with your request, we will contact you by phone.
