Adolescent Family Life Program
The Adolescent Family Life Program (AFLP) is a free, voluntary program for pregnant and parenting youth that focuses on the positives in their lives. Working with a case manager, youth will focus on their strengths, build resiliency, explore values, and set goals. AFLP offers challenging and interesting activities where youth explore what is important to them and start to dream of the future they want for their family.
What Services Does AFLP Provide?
AFLP uses the Positive Youth Development (PYD) model to conduct one-on-one home-visits. PYD is a youth-centered, strength-based approach that focuses on the youth’s, needs, interests, and values. The PYD model recognizes that youth have individual strengths and benefit from supportive relationships and can make meaningful contributions to their lives, families, and communities. AFLP services include:
- Individual support from an encouraging case manager
- Connection to helpful community resources for you and your child
- Support with accessing health care and Medi-Cal
- Life Planning and Goal Setting
- Empowerment and Resiliency
- Support with completing school and/or maintaining employment
- Health education, family planning, and healthy relationship support
Who is Eligible for AFLP?
Young people up to age 21 (at the time of enrollment)
Expectant and/or parenting, including partners in pregnancy and parenting
Contra Costa County residents
How Can I Enroll in AFLP?
Anyone can refer pregnant or parenting youth to AFLP or youth can self-refer.

Following the referral, youth will be screened and assessed for AFLP eligibility. If eligibility criteria for AFLP is not met, youth will be considered for other home-visiting support programs in Contra Costa County.
