Grievances & Grievance Form
If you have a problem or are not happy about a service you received, you may file a complaint or grievance.
After you have filed a complaint or grievance, you will receive a Grievance Acknowledgement Letter within five (5) business days to inform you that your grievance has been received. CCS will review and resolve your case within thirty (30) calendar days.
Ways to File a Grievance

Call the Contra Costa California Children's Services Program, Monday – Friday, 8:00 am–4:30 pm at 1-925-655-0200 and ask for the grievance liaison to submit a verbal grievance

Send an email to the CCS office CCS@cchealth.org

Write about the issue in a letter or on a Grievance Form that you can download and print. Mail or fax it to:
