Who Can Join
Full-time, permanent employees, and their dependents qualify for Plan A and Plan B through CCHP.
County Temps and permanent Intermittent Employees and their dependents may qualify for Plan A2 if they meet specific eligibility requirements. (Call County's Benefits at 925-335-1740 to find out the eligibility requirements for Plan A2)

- Community Provider Network (CPN): Private community doctors and medical providers
- Small hospital co pay
- Small outpatient co pay
- Small prescription co pay
- World wide emergency and urgent care coverage
- Acupuncture and chiropractic care
- Advice Nurse services 24 hours a day, every day

What You Pay

What You Need to Do
Plan A and Plan B and Plan A2 eligible employees need to call the County Benefits Office at 925-335-1740 to enroll or follow the directions provided during open enrollment.

Plans A, B, and A2 employee members should call Contra Costa Health Plan's Member Services department at 877-661-6230 Press option 2 between 8 a.m. and 5 p.m. Monday through Friday.
