Older Adult Mental Health provides a continuum of care to seniors throughout Contra Costa County with multi-disciplinary care teams that provide outreach, engagement and ongoing support services. Care teams include a geropsychiatrist, nurse, mental health clinicians and a community support worker. We collaborate with family members and other service providers to deliver client-centered care.
We primarily offer care in the field where people live. Our Intensive Care Management Program provides field-based services to older adults (60+) diagnosed with chronic and severe mental illness in their homes, board-and-care homes, shelters, etc.
The IMPACT (Improving Mood Providing Access to Collaborative Treatment) Program provides short-term cognitive behavioral therapy and problem-solving therapy to older adults (55+) who often have co-occurring medical diagnoses, such as diabetes, cardio-vascular disease and/ or chronic pain, as well as moderate to severe depression, anxiety or post-traumatic stress disorder.
The Senior Peer Counseling Program provides lay peer counseling services to older adults (55+) who have mild mental health issues such as depression, grief and loss, social isolation, etc. The counseling services are provided by trained volunteer counselors who are who are age 55+. These counseling services are provided in the client’s home, senior center, or community.

- Senior peer counseling to support individuals experiencing mild mental health concerns
