Eliminating Health Disparities
Welcome to Eliminating Health Disparities! In partnership with the Center for Human Development, Contra Costa Regional Medical Centers & Health Centers offers culturally and linguistically appropriate health navigation (assisting community members and patients with applying for public benefits, assistance with medical appointments, food, transportation), and linking patients to community resources, support and advocacy, as part of our efforts to eliminate health disparities and healthcare disparities. The Eliminating Health Disparities Unit oversees these efforts.
What are Health Disparities?
The differences in a health burden(s) among different populations, including differences by gender, race or ethnicity, education or income; or living in various geographic locations.
Our Guiding Principles
We are committed to equity and eliminating health disparities because the department’s mission is to care for and improve the health of all who live in Contra Costa County with special attention to those who are most vulnerable to health problems. Disparities based on race, ethnicity, language, socio-economic status or other similar reasons are inconsistent with our mission.
We are committed to being respectful and responsive to all people we serve and with whom we work. This means we serve people in settings in which they can feel safe and comfortable; we provide services without discrimination and with respect for cultural and language differences; and we respect each other.
We recognize that differences in race, ethnicity, age, gender, sexual orientation, language, physical ability, socioeconomic class, education and many other factors can affect how we relate to patients, clients, customers, communities and each other.
We recognize that beyond our differences lies a common purpose to work together to improve health.
