The Health Care Program for Children in Foster Care (HCPCFC) is a public health nursing program that provides care coordination for medical, dental, behavioral, and developmental services for children in foster care. Public Health Nurses (PHNs) collaborate with EHSD Children and Family Services (CFS) social workers, Juvenile Probation Department probation officers, the medical community, and foster families to ensure children in foster care receive medical services.

How the HCPCFC program can help:

  • Assistance with medical and dental appointments
  • Coordination of care for complex medical needs
  • Medical record review to assess needs of foster youth
  • Linkage to developmental services
  • Monitoring of psychotropic medications and court approval
  • Updating child’s foster care team with health information
  • Providing advocacy and support for foster child
  • Serving as a resource to social workers, probation, foster parents and community health partners
