How to Sign Up for MyChart

You have a few different options to activate your own MyChart account.

  • Self-signup: Go to the signup page on the MyChart website and fill out the requested information. Once we match the information with our records, you will be on your way!

  • At your doctor's office: All Contra Costa Regional Medical Center & Health Centers patients receive a unique MyChart activation code in the "After Visit Summary" paperwork given to them following an office visit. Go to the MyChart website and follow the step-by-step sign-up instructions in your After Visit Summary.

  • Contact MyChart customer support: Call us toll-free (1-844-622-5465) or send us an email at saying you'd like to start a new MyChart account. After we confirm your identity, we will send you an email with an activation code. That's it!
