Program Description

Older Adult Mental Health provides a continuum of care to seniors throughout Contra Costa County with teams that provide outreach, engagement, and on-going services to the most imperiled seniors in the county.

Our Intensive Care Management Program consists of three multidisciplinary teams who provide community-based services to individuals diagnosed with a chronic and severe mental illness in their homes, board and care homes, shelters, etc. The Intern will be working with a multi-disciplinary team, which includes a Geropsychiatrist, Nurse, Mental Health Clinicians and a Community Support Worker. The intern will gain experience in several areas, including providing case management services to older adults, collaborating with family members and other service providers, and providing crisis intervention/risk assessment as needed. In addition, there can be the opportunity to complete intakes, and provide supportive therapy.

Older Adult Mental Health integrates the IMPACT Program model in primary care clinics in each region of the county. Short-term cognitive therapy is provided in the clinics to older adults with medical diagnoses of diabetes, cardio-vascular disease, and/ or chronic pain and who additionally show signs of mild to moderate depression and possible suicide risk.

Partners in Aging is a new program funded through Mental Health Services Act Innovation Funds designed to expand the IMPACT Program. This program consists of a multi-disciplinary team, which includes a Geropsychiatrist, Mental Health Clinicians, and a Community Support Worker. The intern will gain experience in several areas, including providing intakes and therapy to older adults using the IMPACT model, and collaborating with Psychiatric Emergency Services and other outpatient clinics to provide outreach and engagement. Consultation with Alcohol and Other Drugs Services, referrals to community agencies, education, and collaboration with family members and natural supports will also be part of the job duties. This is a field-based position, which will be primarily located in Central County, but will require travel to East and/or West County regions.

A stipend will be provided by Contra Costa County.

Bilingual interns are especially encouraged to apply.
