Claims Information


Submitting Claims

Contra Costa Health Plan is regulated by the Department of Healthcare Services to process claims within 45 business days of receipt. If you are a non-contracting provider and have never submitted a claim to CCHP or within the last 2 years, please submit a current W9, along with the claim and supporting documents to ARSupport@cchealth.org for faster claim adjudication.

Additionally, you may now submit claims and receive payments electronically. Electronic Data Interchange (EDI) and the Provider Portal allow you to exchange healthcare information in a safe, secure, and cost-effective way.
