Submitting Claims

Contra Costa Health Plan is regulated by the Department of Healthcare Services to process claims within 45 business days of receipt. If you are a non-contracting provider and have never submitted a claim to CCHP or within the last 2 years, please submit a current W9, along with the claim and supporting documents to for faster claim adjudication.

Additionally, you may now submit claims and receive payments electronically. Electronic Data Interchange (EDI) and the Provider Portal allow you to exchange healthcare information in a safe, secure, and cost-effective way.
