Coronavirus (COVID-19) for
Congregate Living Facilities
The Contra Costa County Residential Care Facilities (RCF) Outbreak Team monitors outbreaks in Hospitals, and congregate living facilities, including Skilled Nursing Facilities, Assisted Living/Memory Care Facilities, Elderly Group Home/Board & Care, Intermediate Care/Developmentally Disabled Facilities, Sober Living Environment, Homeless Shelters, Receiving Centers, and Youth Group Home/Board & Cares.
All positive test results for SARS-CoV-2 (COVID-19) of residents and staff shall be reported by the facility immediately to Contra Costa County's Outbreak Team. Use the Shared Portal for Outbreak Tracking (SPOT) to report case(s) of residents, staff, and any suspected outbreaks in congregate living facilities, hospitals, homeless shelters, etc. Once your SPOT Intake Form is submitted, a confirmation email will be sent to the facility's point of contact. A public health investigator will contact the facility the following business day to provide guidance and the next steps.
- SPOT Reporting
The SPOT Intake Form is a place for congregate settings and other locations to report basic information about an exposure at their location. You will be asked to enter information about the exposure at your location. Once the information you enter is reviewed by the health department, you may be provided additional login credentials to enter specific data about persons involved in the exposure. Use the Shared Portal for Outbreak Tracking (SPOT) to report COVID-19 positive case(s) of residents, staff, and suspected outbreaks in congregate living settings. Once your SPOT Intake Form is submitted, a confirmation email will be sent to the facility point of contact. - Roster Reporting
Reporting through Shared Portal for Outbreak Tracking (SPOT) is always the preferred method to alert the county of a suspected outbreak. For roster submission, RCF has an alternate reporting method via RCF’s online Intake Form. Once the Intake Form is submitted, a confirmation email will be sent to facility point of contact with COVID-19 Outbreak information.
