Reporting a Positive Case


The Contra Costa County Residential Care Facilities Outbreak Team monitors outbreaks in congregate living facilities including Skilled Nursing Facilities, Assisted Living/Memory Care Facilities, Elderly Group Home/Board and Care, Intermediate Care/Developmentally Disabled Facilities, Sober Living Environment, and Youth Group Home/Board and Care. COVID-19 positive case(s) of residents and staff must be reported to the RCF Outbreak Team via the online RCF Intake Form. Once the RCF Intake Form is submitted, a confirmation email will be sent to facility point of contact with COVID-19 Outbreak information. A Public Health investigator will contact the facility the following business day.
