Member Handbook
Has information on what benefits are covered, how to use services, your rights & responsibilities, and how to file a complaint.
You may request a copy of the Beneficiary Handbook by:

To file a written complaint/grievance, please download the consumer grievance review request form below or you can obtain a form at any County location providing specialty behavior health services.
For assistance completing the form, you can contact a Patient Rights Advocate at 925-293-4942 or 844-666-0472.
