Out of Hospital (Home) Births

If your baby was not born in a hospital you must call 925-313-1125 to schedule an appointment to register your child in person. The process can take approximately one hour and is by appointment ONLY.

You may download the Contra Costa County - Out of Hospital Birth Packet here that explains the evidence you will need to gather for your appointment. If you had a midwife in attendance they will need to sign the Affidavit in the packet.

The Contra Costa County Homebirth Worksheet must also be fully completed PRIOR to your appointment.

There is no fee to register the baby, however a certified birth certificate is $32.00
