Health Care for the Homeless

Our Mission

To improve the health care status of the homeless in Contra Costa County by providing accessible, culturally sensitive, non-traditional clinics in the community and to assist the homeless with access to the traditional primary care system.

About our services

Health Care for the Homeless is a multi-disciplinary bilingual team of medical, social support and administrative professionals. This team provides:

Urgent medical care
TB testing
Medi-Cal and Basic Adult Care eligibility assistance
Mental Health/Substance Abuse support and linkage

Health Care for the Homeless is outfitted with a well-equipped medical van that visits various shelters and community centers through Contra Costa County on a weekly basis. Our services are available to the general public. No appointments are necessary.
