Child Health and Disability Prevention (CHDP) Program

CHDP is a preventive health program, working to make health care available to all children who are uninsured or under-insured. CHDP works with families to:

  • Obtain regular preventive health assessments for infants, children, and adolescents,
  • Coordinate follow-up of identified health problems, and
  • Refer those with suspected health problems for necessary diagnosis and treatment.
  • Help children find medical and dental homes for their health care

What is the CHDP Gateway?

The Department of Health Services has developed an electronic "gateway" at CHDP provider offices and clinics to link children with temporary full scope Medi-Cal coverage.

The Gateway offers preventive care for children, care for children when they get sick, pre-enrollments in comprehensive health insurance, and a doctor that knows your child (a medical home).

Who is Eligible for the CHDP Gateway?

Children and youth who
  • do not have medical insurance
  • are under 19 years of age
  • live in California
  • have family incomes at or below 200% of federal income guidelines

Children already enrolled in Healthy Families and/or full-scope Medi-Cal are not eligible for the Gateway. If you child is not eligible for Medi-Cal or Healthy Families, he/she may continue to get well-child exams at no cost through the CHDP Program.

How does the CHDP Gateway Work?

Step 1:
The provider's office talks to the client about health insurance.
- If client does not have insurance, the parent/guardian completes a CHDP pre-enrollment application.
- If client has insurance, provider bills accordingly or refers client to an appropriate provider

Step 2:
The CHDP provider enters the client's data into the computer and temporary Medi-Cal eligibility is effective immediately

Step 3:
If the parents complete their paperwork within two months and their family is eligible, their children will continue to receive Medi-Cal.

If eligible for temporary full-scope Medi-Cal, the child receives a CHDP exam and temporary full-scope Medi-Cal for the month seen and one month after. An application will be mailed to the client's home for on-going Medi-Cal.

What does a CHDP Well-Child Exam Include?

  • Health and developmental history
  • Physical examination
  • Nutritional assessment
  • Immunizations
  • Dental evaluation & referral
  • Vision and hearing testing
  • Tuberculin Skin Testing
  • Laboratory tests (including sickle cell, blood lead screening, urinalysis, hemoglobin/hematocrit, pap smears, sexually transmitted infections)
  • Health education
In addition, CHDP provides health exams for:
  • Children starting kindergarten or first grade who need a health exam to enroll in school
  • Children involved in sports who need "sports physicals"
  • Foster children
  • Children with a history of perinatal problems
  • Children with significant developmental disabilities

How Can a Child Obtain a CHDP Health Assessment?

  • If you have health insurance, call Central Appointments at 1-800-495-8885.
  • If you do not have health insurance, call CHDP at 925-313-6150.
