Program Description

Older Adult Mental Health provides a continuum of care to seniors throughout Contra Costa County with teams that provide outreach, engagement, and on-going services to the most imperiled seniors in the county. Older Adult Mental Health integrates the evidence-based IMPACT (Improving Mood Access to Collaborative Treatment) Program model in primary care clinics in each region of the county. Short-term cognitive therapy is provided in the clinics to a set of older adults (55 and over) with medical diagnoses of diabetes, cardio-vascular disease, and/ or chronic pain and who additionally show signs of moderate to severe depression and possible suicide risk. Partners in Aging is a new program funded through Mental Health Services Act Innovation Funds designed to expand the IMPACT Program. We are seeking 2 Interns for the Partners in Aging Program. These Interns will screen IMPACT clients for alcohol misuse by utilizing the SBIRT (Screening, Brief Intervention, Referral to Treatment) tool, and offering SBIRT services to those who screen positive for moderate misuse. Consultation with Alcohol and Other Drugs Services, referrals to community agencies, education, and collaboration with family members and natural supports will also be part of the job duties. This is a field-based position, which will be primarily located in Central County, but will require travel to East and/or West County regions. The Interns will join a multi-disciplinary team, including Psychiatrists, Community Support Workers, and Mental Health Clinical Specialists. A stipend will be provided by Contra Costa County.
