Homeless youth – health, outreach, and peer education
HY-HOPE services are intertwined throughout the Youth Continuum and are available to all youth accessing our services.
- Healthcare is provided through an adolescent health clinic on-site at Calli House. Sponsored by Health Care for the Homeless, a nurse practitioner provides health assessments, physical exams, immunizations, STD testing, and family planning services. All services are provided confidentially and free of charge to homeless youth.
- Outreach is conducted weekly to homeless youth living on the streets and youth at-risk of homelessness. Our team provides direct access to shelter services, critical information, referrals to needed services, health and safety information, crisis intervention and counseling.
- Peer Education is interwoven into all our services and allows youth to provide support and education to each other. Current and former CCYCS youth have the opportunities to lead support groups, life skills education classes, conduct outreach and participate in community service activities.
Our Youth Navigator provides transportation and assistance in gaining access to services in the community. The youth navigator works with all the youth in our programs to prepare them to live independent and productive lives. Offering one-on-one life skills education, life skills groups and ready to rent groups the Youth Navigator supports the youth in our continuum through every step of their journey.
Aftercare Services are also offered to all youth who transition out of the program and into independent housing or reunify with family. Services include on-going case management, individual and family counseling, use of our drop in center, referrals to services and opportunities to participate in community service and recreational activities. Our aftercare services allow youth to stay connected to our staff the and the relationships they have established while living in our shelter and transitional housing program.
