What You Need to Do
Call CCHP Marketing office at 800-211-8040, for current premium information and submit your completed application, with a check for the first month's premium, application fee and the required medical information to the address listed on the instruction sheet (click below for application and instruction sheet).
Important Note: All applications will be reviewed for pre-existing medical conditions. If your application is denied due to pre-existing medical conditions you will receive information on how to apply for our Major Risk Medical Insurance Program (MRMIP).
