CCHP Healthy Families Program


Contra Costa Health Plan welcomes former HealthNet members to our Healthy Families program.
Applicants please call our Marketing Office at 800-211-8040 and we will be glad to answer any questions you may have about joining our Plan. Contra Costa Health Plan is working with former HealthNet providers to assure continuity of care, when you join our Health Plan.

Who Can Join

If your children live in Contra Costa County, and meet the income guidelines (see below) and meet the following qualifications, they are eligible for the Healthy Families Program:

  • Not currently covered by an employer-sponsored health plan
  • Have not been covered by an employer sponsored health plan in the last three months
  • Are not eligible for or enrolled in no-cost Medi-Cal
  • Are less than 19 years of age,
  • Are U.S. citizens, nationals, or qualified immigrants

  • Low co-pays and no paperwork
  • Advice nurse services 24 hours a day
  • Access to 2 provider networks
    • Contra Costa Regional Medical Center and 8 neighborhood health centers
    • Private Community Doctors and medical providers

What You Pay

Contra Costa Health Plan (CCHP) has the lowest monthly premium of all your Healthy Families options. CCHP is the Community Provider Plan in Contra Costa County.

Call Contra Costa Health Plan at 1-800-211-8040 for more information and application assistance. Call the State Healthy Families Program toll free at 1-888-747-1222 for a list of Application Assistance Sites near you.
