California Environmental Reporting System (CERS) Instructions

All regulated businesses are required to submit/review information regarding the annual Hazardous Materials Business Plan, Hazardous Waste Generator Reporting, Underground Storage Tanks, Aboveground Storage Tanks, and/or California Accidental Release Prevention Program (CUPA data) annually. In the past, CCHSHMP sent all regulated businesses a CUPA packet with paper forms to submit. CCHSHMP will no longer be sending paper forms. State law (Assembly Bill 2286) now requires your business to report this information electronically. Contra Costa Health Services Hazardous Materials Programs (CCHSHMP) can no longer accept paper submittals. The State of California has established an electronic information management system known as the California Environmental Reporting System (CERS) to replace paper submittals. Therefore, all regulated businesses MUST use CERS to submit their 2014 CUPA data on or before March 3, 2014.

If you elect not to use instructions provided by CCHSHMP, please note you are required to submit through CERS the following CCHSHMP -specific forms:

  1. Hazardous Waste Generator Program:
    Hazardous Waste Generator Reporting Form
    - Instructions
  2. Aboveground Petroleum Storage Act Program:
    APSA Facility Statement
    - Instructions

PDF fillable forms can only be accessed using Internet Explorer or Chrome web browser.
